Forms

Please log into your MyCBE Account and complete the following forms online. Please complete all forms by September 13, 2024. 

Student Demographic Form 

The information on this form is used to contact parents and guardians. It is VERY IMPORTANT that the school has the most current information for emergency purposes. Any changes to this information throughout the school year should be updated. 

Annual Authorization (2 forms) 

Consent for CBE Use of Student Information by News and Media and Outside Groups (Third party)- this form allows the CBE to use student information for non-profit educational purposes such as public displays and presentations. CJFS and CBE websites and social media, brochures, invitations, reports and newsletters and videos. 

Consent for CBE Use of Student Information- This form allows Colonel J Fred Scott School to share information with parents by highlighting the school, student and student work or activities on our school webpage and CBE website or non-profit educational purposes. We are not permitted to do so without consent. 

Annual Volunteer 

All volunteers are required to complete the CBE Annual Volunteer form. You must also hold a Calgary Police Services Clearance to volunteer. If you need to apply for police clearance, please contact our school office to make an appointment. The clearance costs $21.25 and is valid for 5 years. 

The Student Health Plan form is available upon request. If a student has a significant health concern that may affect their attendance at school, this form is to be completed and returned to the office. The Student Health Plan is also used when the student is asked to take medication during school hours or activities. 

Student Fees 

Student Supplies fee 

A fee of $40 for Grade 1-6 students and $20 for kindergarten students is charged to cover all student supplies like pencils, notebooks, glue, crayons, etc. You can pay for these fees starting October 15. 

Lunchroom Supervision 

All students from Grade 1-6 who want to stay for lunch can start registering October 15, 2024. The annual fee for the 4-day lunch program will be $305. This fee covers the supervision for students during the noon hour; this does not include food or drink. 

Lost Library Book 

If your child does not return a library book after its due date, FIRST NOTICE will go home 2 weeks after the due date stating the cost of the book if it is not returned. A SECOND NOTICE will be sent home 4 weeks after the due date, notifying that the book is still missing. The cost to replace the book will now be charged on your MyCBE Account to be paid. Payment online is preferred. After 4 weeks that book is yours. 

Field Trip Fees 

We are fortunate that our School Council help pay for fieldtrips throughout the year. Sometimes a classroom will still need to charge a fieldtrip fee but it will not be more that $10.
